Not sure about that. I think he realises that he accidentally hurt her, but he tries to make light of it, which might not be brilliant, but I don't see it as malicious.

Re. the balcony scene, then two people have expressed themselves far better than I, so I shall resort to quoting:
His first error happens at the Bronze when he talks to Buffy about how her friends would respond if they knew about her relationship with him. This feeds Buffy's fear and leads to further polarization of her attitude toward Spike. Spike does this because he intuitively realizes it is a hot point for her. He's striving for an emotional response, recognizing that his major advantage is the ability to evoke emotion in Buffy, make her feel alive. Something she later confesses to Tara, saying she only feels when with Spike. But this is the wrong emotion. He evokes her growing sense of guilt, which leads to her decision to turn herself in.

"You always end up in the dark with me," says Spike to Buffy in Dead Things, and it's a hollow victory. Fighting to make her give in to the darkness weighing on her was never what this was supposed to be about. She was his inspiration to fight his own darkness, and if he knew how to fight for her, to take on the darkness in her as well as in him, I think he would.

I don't think he would ever want to kill her, but he is really trying to get her to accept him as more than just a convenience - he is the one who constantly wants to talk about the relationship. (Although I don't think it could ever work as long as he is soul-less.)
